You may register early by email or mail for any class, prior to the walk-in registration dates. Do not mail cash. Make checks or money orders payable to UCA CSM. Mail payment with registration form to: UCA CSM, 201 Donaghey Avenue, SFAC 203, Conway, AR 72035. Payment is due at registration and in advance of the first class. If you should need a deferred payment plan, please contact the CSM office for approval before mailing in your first payment. IMPORTANT: Past due accounts must be paid in full before the beginning of each semester.

PARKING PERMIT PROCEDURES
A parking permit is required each semester and allows you to park in any designated Visitor, Staff, Student, or UCA parking area. Take your CSM receipt to the UCA police station to receive your free permit. The police dept. is open 24 hours a day, 7 days a week and is located one block directly west of SFAC.
ABOUT TUITION PAYMENT, REFUNDS AND MAKE-UPS
TUITION:
Class and lesson fees are paid in advance on a semester basis. If requested, a deferred schedule of payments may be allowed but we must have a Multiple Payment Form on file and at least 1/3 is due at registration, 1/3 by Sept. 15, and 1/3 by Oct. 15. A two-payment schedule may also be arranged. Students who begin lessons the second week or later will pay a pro-rated amount.
REFUNDS:
A full refund, less the non-refundable $15.00 registration fee, will be given if cancellation of enrollment is made prior to the first class or lesson. You must contact the CSM director in person or in writing. A partial refund will be given after lessons begin, based upon the following requirements: private lessons – prior to the 3rd lesson; group lessons or ensembles – prior to the 2nd class. After the deadline, refunds on private lessons may only be given for medical reasons or if student moves out of the area. Refunds will not be given for group lessons or ensembles if the second class has met.

MAKE-UPS
Please note our make-up policy. Teachers are required to make up any lessons they cancel. Teachers are not required to make up lessons cancelled by their students. Students are responsible for attending all lessons for which they register and must notify the CSM office (450-5755) or their teacher if they will not be able to attend a lesson. Please request your teacher’s phone number and email address in case you cannot immediately contact someone in our office.
UCA is closed for Labor Day, however, the Snow Fine Arts Center will be open and some insructors may teach on both Labor Day and during Fall Break. If your lesson is on Labor Day or falls on October Break, please confirm with your instructor whether lessons will be taught. Lessons falling on Labor Day that are canceled by the teacher will be made up Nov. 24 or earlier. Lessons during Fall Break (Oct. 16-17) will be made up when convenient.
COMPLETE THE REGISTRATION FORM AND MULTIPLE PAYMENT FORM IF NEEDED AND MAIL WITH YOUR FEE PAYMENT TO: UCA Community School of Music, SFAC 203, 201 Donaghey Ave., Conway, AR 72035. Please make your check payable to “UCA CSM.” If more than one family member is registering, please use a separate form for each student. You may copy this form. Please note your preferred lesson time and instructor if possible. The director or your teacher should contact you about your lesson time prior to the first week of lessons.
