Notes on the Faculty Evaluation Survey Report
In order to safeguard the anonymity of the student, no report will be issued for any course section with fewer than five student responses to the survey.
Left: Instructor's Name, Instructor's UCA ID, Department,* College
* The "department" field is intended to indicate the department (or non-departmental program—e.g., the MBA program or the Intensive English Program) where the teaching occurred.
Right: Course Sequence Number, Program Prefix,* Course Number, Number of Respondents
* The "prefix" field is generally based on the course information sheet provided by the department or college; for that reason, it may or may not reflect the actual program prefix for the course.
The body of the report is in two parts:
(1) In the left column is the score report. The distribution of responses is shown for all items. In addition, the instructor's mean scores are shown for items 1–3. To make calculation of mean scores possible, the following values are assigned: A = 5, B = 4, C = 3, D = 2, E = 1.
(2) In the right column, the instructor's mean scores for items 1–3 are plotted in relation to the following information about the mean scores on these items of courses in the college or department:
For each course, comparison is made only with courses from the same level (lower division, upper division, graduate). Note that if a context group (department or college) contains fewer than five courses at a particular level, full contextual information for that group at that level is not plotted.
