REGISTRATION

In order to register with Disability Support Services (DSS) students must: 

• Complete DSS registration forms.
• Submit documentation of disability.
• Schedule an appointment with Disability Support Staff.

You may print these documents (pdf format) or request copies from DSS.

Acrobat Reader® must be installed on your computer to view or print these documents. This is available free from Adobe or, on campus, from the UCA web server.
