Complete all forms neatly, completely and accurately

Application for Graduate Degree:  Please print VERY clearly in black ink as this information will be used/printed EXACTLY as it appears on this form.  If you will be submitting a thesis or dissertation contact Susan Wood in the Graduate School office before completing your application.

Change in Petition for Candidacy:  This form is only required if the courses you complete for your degree vary from those listed on your approved Petition for Candidacy / Program of Study.

AFTER TURNING IN THE PACKET

A letter from the Graduate Office will be mailed to you with information regarding the Commencement Ceremony.

All students receiving financial aid must contact the Financial Aid Office at 450-3140 to schedule an exit interview.

For information concerning caps and gowns, graduation invitations, and class rings, contact the UCA Bookstore at (501) 450-3141.

Diplomas will be mailed approximately 10-12 weeks after graduation.  For information concerning Teacher Licensure, contact Dr. Ken Vaughn, Mashburn Hall Room 119A, 450-3131.
