Does this group life insurance program provide whole life insurance or term insurance?

  • Answer: Term insurance.

What is the maximum “basic” coverage available to me?

  • Answer: UCA provides one times your annual salary (rounded to the next thousand). UCA pays 100% of the premium with  a maximum of $250,000.

What is the minimum “basic” coverage available?

  • Answer: $25,000

What is the effective date of my “basic” life insurance coverage?

  • Answer: It becomes effective the 1st day of the month following 90 days of benefits-eligible employment.

How much “optional” life insurance my I purchase?

  • Answer: The employee may purchase one, two, or three times their annual salary (rounded to the next thousand) in “optional” life insurance coverage. The employee pays 100% of the premium.

What is the maximum “optional” coverage available?

  • Answer: $350,000

What is the effective date of my “optional” life insurance coverage?

  • Answer: It becomes effective the 1st day of the month following 90 days of benefits-eligible employment.

May I purchase life insurance coverage for my spouse and/or dependent children?

  • Answer: No. The UCA group life insurance plan only insures the employee.

Can I name a minor as my beneficiary on the life insurance? If so, who is in charge of the money until the minor reaches 18?

  • Answer: Yes, you may name a minor as the beneficiary. The money is put into a trust account with MetLife. The child will have full access to the funds at age 18.

When can I change my beneficiary?

  • Answer: You may change your beneficiary at any time by completing the beneficiary change form in the UCA Human Resource office.

If I change my beneficiary, when does the change become effective?

  • Answer: Immediately.
