Insurance Coverage Equal to a $500 Deductible - GAP Insurance

Q. What is GAP Insurance?

A. GAP insurance is the self-funded supplemental form of insurance that UCA provides free to UCA employees to off-set the $1,000 deductible on the UCA health insurance plan.

Q. Who is eligible to receive GAP?

A . GAP is only available to full-time, benefits-eligible employees, who have purchased UCA’s group health coverage.

Q. Is GAP available to my spouse and children?

A. Yes, but it isn’t provided free of charge.  There is a monthly charge and you must purchase it when signing up for the UCA health insurance. 

Q. How much of my deductible will GAP cover?

A. After you have reached the first $500 of the $1000 deductible on UCA’s health coverage, GAP will reimburse the amount you go above that first $500 of your deductible up to a maximum of $500 per calendar year.

Q. Will it cover up to $500 per person on family coverage?

A. No. If you have purchased GAP coverage for two-party or family coverage the maximum reimbursement is $1000.  Again, it reimburses the 2nd $500 of the deductible on two people.  A family of 2 or more would receive a maximum reimbursement of $1000 per calendar year.

Q. I have a $1000 deductible to pay but don’t have the money right now. Will GAP pay the first $500?

A. No. GAP does not apply to the first $500 of the $1000 deductible, only to the second $500. The employee is responsible for the first $500. If your provider requires payment in full, the employee is responsible for full payment, and UCA will reimburse the second $500.

Q. What do I need to do to get GAP reimbursement?

A. An Explanation of Benefit Form (EOB) will be required from United Health Care for reimbursement. A claim form is available at the Human Resource website link.  Employee(s) must complete the GAP claim form and submit it to the UCA Human Resource Office, Wingo Hall Suite 106.

Q. I have some GAP reimbursement receipts. Who do I need to give these to?

A. Contact the Human Resources Department at UCA. Wingo Hall Suite 106
