Have a current unencumbered LPN or LPTN license and have a copy of the license to submit with the application.

Meet GPA and course prerequisite requirements.

If not currently a student at UCA, have submitted an Undergraduate application to UCA.

Have requested hard-copy (paper) transcripts of any courses not taken at UCA to be sent directly to the Department of Nursing.

________

Have submitted the Work Reference form (with a stamped-envelop containing the Department of Nursing's address) to an individual who is my direct clinical supervisor.

Have obtained the Verification of Clinical Practice form (from all clinical sites needed to meet the articulation requirements outlined in the admission criteria) and have ready to submit with the application.

Have read the following documents.

• Abilities and Skills for Admission to and Progression
• Substance Abuse Policy

Have completed and submitted the LPN/LPTN-to-BSN application form with all required supporting documents to the Department of Nursing between January 15 and March 1.
