Meet GPA and course prerequisite requirements.

If not currently a student at UCA, have submitted an Undergraduate application to UCA.

Have requested hard-copy (paper) transcripts of any courses not taken at UCA to be sent directly to the Department of Nursing.

Have completed and have ready to submit with the application a one-page typed (double-spaced) narrative addressing my future goals and how my studies at UCA will help me fulfill these goals.

Have completed and have ready to submit with the application a brief resume summarizing my educational, work, and professional experience.

________

Have submitted the Work Reference form (with a stamped-envelop containing the Department of Nursing's address) to an individual who is my direct clinical supervisor.

Have obtained the Verification of Clinical Practice form (from all clinical sites needed to meet the articulation requirements outlined in the admission criteria) and have ready to submit with the application.

Have obtained the Clinical Background Check verification form and have ready to submit with the application.

Have completed and submitted the RN-to-BSN application form to the Department of Nursing by the due date that corresponds with the semester I want to begin class:

• Fall admission: between February 15 and April 1
• Spring admission: between June 15 and August 1
